The first step towards a beautiful, healthy smile for your child is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If your child is a new patient to our office, please visit our Patient Forms page  to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your child's medical needs more quickly than completing them on your arrival. Payment Options

Payment at the time of treatment is advised. For your convenience, we accept Visa, Amex, and Master. We take cash and credit card. We can also arrange in-house financing for uninsured patients. Please feel free to ask our team.  

Dental Insurance

We are in-network providers with many of the PPO insurance plan  and medicaid. If you do not see your insurance plan, please give our office a call for Special Plan at (973) 500-4144 and we will be more than happy to verify your coverage. Our office will make every effort to provide you with the finest care and the most convenient financial options.                                                                                                                                                     
To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures. Our policy is to submit all insurance claims electronically on a daily basis. If you have questions regarding your insurance, please contact us at (973) 500-4144.
